** Has your dental insurance recently changed? **
Please be sure to let us know PRIOR to your dental appointment if you've had a change
in insurance carriers or plans. We will be happy to call and determine your benefit coverage for you.
~NEW PATIENTS ONLY~
For our new patients our policy is to appoint you for a comprehensive evaluation and necessary radiographs to assess your particular dental needs on your first visit to the office. You will be reappointed for your dental cleaning. We do this for the following reasons: we need to how best to treat you and allow the proper time to accomplish that goal; we want to get acquainted with you and hopefully begin a long relationship and we want to let you see our office and meet our staff so you can assess if this is the office for you. Also, you can let us know your expectations of our office and staff and answer questions you may have regarding your dental needs. Once we establish you as a patient of record, your semi-annual visits will include your examination, cleaning and radiographs (if necessary).
